- What is the date on which you would like to receive service?
- What is the time of the appointment or need to be at destination?
- Contact information if other than the person scheduling the ride?
- Emergency contact information?
- Is the person in a wheelchair/ or ambulatory?
- Are there any steps involved at either destination?
- Is the person traveling with oxygen?
- Is there and aid traveling with this person?
- Does this person require from us a wheelchair?
- Does the person require any other special needs during transportation?

- If going to a Doctor’s office, the name of the doctor being visited
